This case map addresses ACOTE standard two. ACOTE standard two looks at using evaluations to come up with different plans and treatments. Specific models as well as assessments were used to develop a an effective and robust treatment plan for this case. This case map also addresses an ability to plan appropriate and effective treatment plans, with creativity and efficiency noted by professor.

This SOAP note focuses on ACOTE standards four and five. This artifact addresses documentation of a community client therapy session, as well as interaction with the caregiver and the client. This SOAP note was the final SOAP note written, and it demonstrates improved clinical reasoning skills, as well as development of positive interaction and appropriate documentation.

This pediatric community client treatment plan centers on ACOTE standard number six, which states: "Demonstrate the ability to plan, develop, organize, and market the delivery of services to include the determination of programmatic needs and service delivery options and formulation and management of staffing for effective service provision." This community client treatment plan shows development and organization of services and how those services will be laid out in treatment sessions.

The situation is highlighted with descriptions of what took place, as well as ethical dilemmas experienced by the student therapist during the session. This artifact addresses ACOTE standard eight. This artifact represents this standard as it focuses on specific ethical concerns and ways that they could be resolved, and potential concerns related to theses solutions.

This artifact addresses ACOTE standard number one, as this artifact centers on specific evaluation of a community client. The client was evaluated using multiple assessments, as well as observation of ADL and IADL tasks. The client was also interviewed about specific therapy goals and desires of treatment. These observations, and the information that was gathered is included in the evaluation documentation. Although there are multiple comments and references to fix content, this is a good representation of taking constructive feedback and implementing it. I was able to use the feedback in further evaluations of clients, as well as to garner more information from my client as needed.

Community Client Adult Semester Discharge Summary
This discharge summary applies to ACOTE standard three. This discharge summary looks at the continuum of care throughout the intervention process, as well as the clients current status, home programs, continued therapy needs, and client goals after discharge. The discharge focuses on long term and short term goals, and whether those goals were met or not. Additionally, the client was asked about their satisfaction with the occupational therapy services received.
